Terms of Service

1. By placing an order, you certify that you are at least 18 years of age and you request and consent to the collection and testing of blood and/or urine samples.

2. The results are for informational purposes only and are not meant to provide diagnostic information. In addition, interpretation of the results shall and will be made by your own physician.

3. United Against Diabetes’s physician will order the tests and review the results, but will not diagnose or treat you.

4. Tests ordered through this service are not meant to be reimbursable by any 3rd party payors (e.g., Medicare, Medicaid, Insurance Companies).

5. All test information will be treated confidentially. No health information that identifies you will be released to any entity or individual other than that required by law.

6. You authorize United Against Diabetes to disclose personal and/or health information that identifies you to those employees, vendors, health care professionals, and other affiliates engaged by United Against Diabetes to conduct operations related to ordering, payment processing, and delivery of results; providing that any such entities will be bound by the same confidentiality obligations.

Refund Process
If for any reason, United Against Diabetes is unable to deliver a valid laboratory result for a test you selected, you will be entitled to a refund. For more information please contact us at

Drug Test Authorization
(If applicable), I understand that results of any drug tests are for informational/screening purposes only and cannot be used for Department of Transportation required testing, court ordered or other legally mandated testing, pre-employment testing or employee testing.

HIV Testing
Before consenting to testing please read the following important information:

1. Purpose. This is a voluntary test performed to determine whether you may have been infected with HIV. This test does not diagnose AIDS. Benefits of being tested include early diagnosis and preventing spread of infection. Limitations of being tested include anxiety and discrimination, if testing positive.

2. Testing Procedure. A simple blood test will be taken. Possible side effects include bruising.

3. Accuracy. An HIV test will be considered positive only after a confirmatory test by the lab; however no HIV test is %100 accurate. Possible errors include:
a. False Positives: The test may give a positive result even though you are not infected. Retesting should be done to help confirm the validity of a positive test.
b. False Negatives: The test may give a negative result even though you are infected with HIV. This happens most commonly in recently infected persons. It
may take at least 12 weeks for a positive test result to develop after a person is infected with HIV.
c. Indeterminate: This may indicate recent infection with HIV and will require retesting at least one later.

4. Disclosure of Positive Test Results. If you test positive, we are required by state law to report those results to the county health department in a confidential manner. The local health department will contact you to help with counseling, treatment, case management, and other services if you need and want them.

5. Confidentiality. Like all medical information, HIV test results are confidential. This means that information on the individual and their test result is kept in a confidential record. Another option is to HIV testing performed anonymously through your local county health department.
